• License required: Yes, State of OR (must be in hand at the time of presentation)
• Certifications required: ACLS
• Applicant must provide documentation of provision of surgical services representative of the scope and complexity of the privileges requested during the previous 24 months (applicants who completed training during the previous year must provide a training log).
