This role works under the supervision of a Qualified Supports Coordinator or Manager and adheres to Person-Centered Planning principles and regulatory requirements.Key ResponsibilitiesAssist with implementing Individual Plans of Service (IPOS)Link, coordinate, and monitor services and supportsHelp individuals maintain entitlements (Medicaid, SSI, etc.)Support crisis planning and resolutionCoordinate health and safety strategies, including discharge planningPromote community inclusion, self-advocacy, and relationship buildingAssist with housing, transportation, and employment resourcesComplete timely documentation and maintain compliance with agency and funder requirementsQualificationsEducation: Bachelor's degree in Human Services (eligible for advancement to Supports Coordinator after one year)OR Bachelor's degree in non-human services fieldOR Associate degree or 60 credit hours plus 2 years of experience with individuals with I/DDStrong communication, organizational, and computer skillsValid Michigan driver's license, insurance, and acceptable driving recordAdditional RequirementsExtensive travel within Wayne County; reliable transportation requiredAbility to work flexible hours and maintain documentation standardsCompletion of job-specific training provided by CLSAbout Community Living Services (CLS)CLS provides person-centered supports for individuals with intellectual and developmental disabilities in Wayne County.
