About this role
As a Support Planner at Beatrice Loving Heart, you work with participants in Maryland's Community First Choice (CFC) program. You partner with each participant and their family to develop, implement, and monitor a Person-Centered Plan that captures what matters to them. You coordinate with service providers, advocate for participants, monitor service delivery, and document everything in LTSSMaryland. This is field-based, professional, case management work — not direct care.
