Suicide and Overdose Fatality Review Coordinator

Southern Indiana Coordinator For The Suicide And Overdose Fatality Review (Sofr) ProgramPosition serves as Southern Indiana Coordinator for the Suicide and Overdose Fatality Review (SOFR) program. Provides hands-on support and technical assistance to local SOFR teams and state-level intervention and prevention initiatives. Assists with local team review meetings and implementation of suicide and overdose prevention activities. Ensures dissemination of high-quality data, promotes SOFR best practices, collaborates with SOFR staff to meet needs of local teams. Requests and collects records for fatality review teams, including medical and law enforcement records. Summarizes case information and facilitates local team meetings.

Tracks record requests and case review progress using Microsoft Excel. Enters case information into the national OFR database. Communicates with local SOFR team coordinators about team successes, challenges, and needs. Assists in writing grant applications and reports for the SOFR program. Establishes collaborative relationships with local stakeholders. Provides training and guidance to local SOFR teams in case ascertainment, review, and data collection.

Assists the Programs Director in overseeing reporting requirements from local SOFR teams. Facilitates collaboration between local OFR teams and local strategic planning efforts. Communicates local SOFR team status and needs to Programs Director. Participates in staff development, self-care, and team building activities. Must have a Bachelor’s degree and knowledge of state and local health agencies. Incumbent does not have any supervisory responsibilities or direct reports.

Requires significant attention to detail, ability to resolve problems, interpret policy and have superb oral and written skills. High level responsibility for ensuring agency compliance with SOFR requirements. Primarily a desk job, but requires statewide travel, with extensive travel in the Southern region of the state. Requires extensive computer use and ability to lift and transport boxes and case materials.
