Seeking a State Licensed Transitions of Care Physician, the full-time remote role focuses on delivering high-quality virtual patient care to recently discharged patients at risk for readmission, while also providing clinical supervision and leadership to an interdisciplinary team. Key responsibilities: Deliver high-quality direct virtual patient care to manage complex transitions of care cases Serve as the final escalation point for clinical questions and oversee clinical outcomes to improve readmission rates Provide supervision and coaching to Nurse Practitioners and lead interdisciplinary rounds for high-risk patients Required qualifications: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree Board certification in Internal Medicine, Family Medicine, or related specialty Minimum 3 years of post-residency clinical experience Strong background in inpatient care and transitions of care Experience leading interdisciplinary rounds and supervising healthcare professionals
