To support kidney health management initiatives, the part-time State Licensed Pharmacist will review medication regimens, address pharmacy referrals, and collaborate with clinical teams to optimize patient care in a remote setting. Key responsibilities Review medication regimens and provide recommendations based on current protocols and guidelines Create written responses to pharmacy referrals regarding medication access, education, and safety Collaborate with care providers to enhance medication management and participate in quality improvement activities Required qualifications Active Pharmacist License PharmD degree from an accredited School of Pharmacy 5 years of clinical pharmacy experience in direct collaboration with healthcare providers Experience in renal patient pharmacy management and value-based care preferred Understanding of CMS Star measures and HEDIS measures
