Providing patient-centered MTM services, the full-time State Licensed MTM Pharmacist will optimize medication therapy through comprehensive reviews and proactive patient outreach while working remotely. Key responsibilities Deliver MTM services, including Comprehensive Medication Reviews and Adherence Monitoring Interventions Conduct proactive patient outreach to explain MTM program benefits and engage hesitant patients Communicate clinical recommendations to prescribers and document all patient interactions accurately Required qualifications Bachelor's Degree in Pharmacy or PharmD from an accredited college Current license to practice Pharmacy in the applicable state(s) Completion of an MTM certificate course Minimum of 1 year of relevant work experience Ability to acquire pharmacist licensure in multiple states if requested
