To support a growing clinical services team, the remote State Licensed MTM Pharmacist will conduct telephonic consultations, perform comprehensive medication reviews, and document care while adhering to HIPAA regulations in a contract-to-potential-FTE position. Key responsibilities: Engage members telephonically and schedule pharmacist consultation appointments for clinical programs Utilize prescription claims data to generate clinical recommendations and assess medication regimens Document patient care notes and participate in departmental meetings to enhance clinical programs Required qualifications: Current and unrestricted Pharmacist license in the state of residence Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services 1+ years of experience working remotely/independently in a cross-functional team environment Intermediate proficiency with Microsoft Word, Outlook, and Excel Ability to work a flexible schedule with rotating shifts between 9:00am and 8:00pm CST, Monday through Friday
