Leading the Transitions of Care program, the full-time State Licensed APP Manager will provide clinical oversight and operational management for Advanced Practice Providers while delivering patient care remotely during standard business hours. Key responsibilities Provide excellent patient care during the 30-day post-discharge period, managing symptomatic patients and reducing preventable readmissions Oversee clinical performance and accountability of the APP team, ensuring targets are met and identifying areas for improvement Conduct bi-weekly one-on-one meetings with direct reports, set personalized goals, and assist with recruitment and onboarding of new APPs Required qualifications 5+ years of clinical experience as an Advanced Practice Provider (NP/PA) At least 1 year of experience managing post-discharge patients 2+ years of management or team leadership experience Active RN and APRN or Physician Assistant license with national certification Ability to work Monday through Friday from 9:30am to 6:00pm EST
