Crouse’s Pharmacy team is hiring a Staff Pharmacist .

Available Schedule(s):

• Part-Time (76 hours in a two week period) - 11 :00pm-7:30am, 3:00pm-11:30pm, with rotating weekends, holidays & other shifts as needed (Current rotation is 7 on/7 off)

Pay Rate: $79.00/hour plus applicable shift differential

Staff Pharmacist Responsibilities:

  Completely evaluates all medication orders against clinically appropriate, patient specific parameters. Resolves any identified issues utilizing hospital approved guidelines or with the appropriate provider. Documents all interventions.

  Initiates substitution of a formulary medication for a non-formulary medication when clinically appropriate.

  Appropriately documents all clinical activities.

  n

• n

  Reviews all orders for IV contrast as provided by Radiology.

  n
n
• n

  Efficiently utilizes both hospital based and pharmacy based computer systems.

  n
n
• n

  Assures that all products are appropriately labeled prior to dispensing.

  n
n
• n

  Participates in maintaining a working stock of medications & supplies.

  n
n
• n

  Ensures proper storage of medications.

  n
n
• n

  Ensures that a patient, nursing unit or department is charged for all medications dispensed/ materials utilized.

  n

• n

  Dispenses a 24 hour supply of unit of use medications to individual patients for all new orders.

  n
n
• n

  Checks unit dose carts filled by pharmacy assistants. Checks prepackaged unit dose medications prepared by pharmacy assistants.

  n

Staff Pharmacist Requirements:

  current New York State Pharmacy License

  n
n
• n

  Written and oral communication skill and interpersonal skills to deal effectively with other health care professionals and hospital employees.
