POSITION SUMMARY
The Licensed Practical Nurse (LPN) - Population Health supports the delivery of proactive, patient-centered care by assisting with care coordination, chronic disease management, preventive health initiatives, and quality improvement activities. Working as part of a multidisciplinary team, the LPN engages patients through outreach, education, and follow-up to improve health outcomes, close care gaps, and promote adherence to treatment plans.
Brief Description
The Licensed Practical Nurse (LPN) - Population Health supports the delivery of proactive, patient-centered care by assisting with care coordination, chronic disease management, preventive health initiatives, and quality improvement activities. Working as part of a multidisciplinary team, the LPN engages patients through outreach, education, and follow-up to improve health outcomes, close care gaps, and promote adherence to treatment plans. The LPN plays a key role in supporting value-based care initiatives, preventive screenings, care transitions, and ongoing management of patients with chronic conditions.
Requirements
- HS/GED required
- Current LPN license in good standing with the Florida Board of Nursing
- 1+ years Clinical experience in an ambulatory care setting preferred
- Current CPR certification required
- Excellent listening and interpersonal skills
- Must be flexible, resourceful, and able to troubleshoot
- Must be able to handle multiple tasks simultaneously and set priorities

- Bilingual (Spanish/English) a plus
