Rome Health is currently searching for a Staff/Clinical Pharmacist .

The Staff/Clinical Pharmacist - Per Diem will ensure the safe and effective use of medications through proper storage, handling, evaluation, preparation, compounding, labeling, dispensing, and administration. The Pharmacist must have in-depth knowledge of medications and their uses. Responsibilities include:

  1. Review prescriptions/orders issued by physicians or other authorized prescribers to ensure accuracy and appropriate medication management. Promote formulary use by consulting with prescribers when non-formulary medications are ordered. Process orders within PIS and assist in maintaining accurate patient profiles and charging. Apply mathematical operations for dosage calculations, formulation, and data analysis.
  2. Monitor patient profiles to ensure rational drug therapy. Review drug allergies, interactions, and duplicate therapies during order entry. Adjust drug therapies as authorized by physicians and review labs to ensure medication appropriateness.
  3. Perform all aspects of patient care in an environment that promotes safety and reduces errors. Support a culture of safety and quality.
  4. Maintain competency in Rome Health's Antimicrobial Stewardship program. Carry out initiatives from Infection Prevention and P&T Committee, including monitoring, reporting, and documenting interventions related to antimicrobial stewardship.
  5. Prepare IV admixtures and extemporaneous parenteral products under aseptic conditions, ensuring sterility, proper packaging, labeling, and storage. Comply with USP <797> standards and serve as an Assigned Trainer for compounded sterile and non-sterile preparations.
  6. Provide education to physicians, nurses, students, and patients on medication use, interactions, side effects, dosage, and storage. Document patient education in medical records, aiming for a minimum of 25 patient educations annually. Respect and address the psychosocial, cultural, ethnic, and spiritual needs of patients and families.

Education, Training, Experience, Certification, and Licensure:

  • Graduate of an ACPE-accredited pharmacy school with a BS in pharmacy or PharmD degree.
  • Current New York State License and Registration to practice pharmacy in good standing.
