Overview
Speech-Language Pathologist (DDD Provider) - FamAli LLC is a Qualified Vendor with Arizona's Division of Developmental Disabilities (DDD). Location: Arizona (statewide).
FamAli LLC provides speech-language therapy services to individuals with developmental disabilities, helping clients improve communication, feeding, and social interaction skills.
Responsibilities
* Conduct DDD-compliant speech-language evaluations (expressive/receptive language, articulation, AAC, feeding/swallowing).
* Develop and implement Individualized Treatment Plans (ITPs) aligned with ISP goals.
* Provide direct speech therapy services in home, community, or facility settings.
* Recommend and train on AAC devices and communication strategies.
* Participate in ISP meetings and collaborate with DDD Support Coordinators.
* Educate caregivers and support staff on communication techniques and carryover.
* Maintain documentation per AHCCCS and DDD standards.
* Monitor progress and revise goals as needed.
* Stay current with Arizona licensure and DDD policies.
Qualifications
* Master's in Speech-Language Pathology
* Arizona SLP license, AZDHS and ASHA CCC-SLP certification (required)
* Experience with developmental disabilities (preferred)
* Fingerprint Clearance Card, driver's license, reliable transportation
* Ability to meet DDD credentialing requirements
