You will assess speech, language, cognition, swallowing, and communication needs; develop and implement individualized treatment plans; document progress; and collaborate closely with an interdisciplinary care team.
This role supports person-directed, dignified care, engages families and caregivers, and contributes to quality improvement while mentoring staff and supporting resident education and safe discharge planning.
