Role Overview: The Speech Pathologist evaluates, develops, and implements individualized treatment plans for patients with speech, language, cognition, voice, and dysphagia deficits in home settings. They demonstrate knowledge across age groups from infants to geriatrics, utilizing diagnostic assessments to determine intervention needs.

Key Responsibilities

  • Conduct assessments and interpret diagnostic results to evaluate patient functioning.
  • Create and modify personalized therapy programs, setting achievable goals.
  • Provide direct therapy services tailored to patients’ physical, emotional, and cognitive abilities.
  • Coordinate with multidisciplinary teams, including families and physicians, to ensure comprehensive care.
  • Document evaluation results, treatment plans, progress notes, and other reports accurately.
  • Participate in program development and supervise clinical interns from educational institutions.
  • Communicate effectively with patients and team members to monitor progress and adjust interventions.

Qualifications & Skills

  • Certification from ASHA (CCC-SLP) or active licensure in the practice state.
  • Preferred 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
  • Strong communication, teamwork, and interpersonal skills.
  • Ability to perform essential functions with visual acuity and sound communication.
