Description
The Speech Language Pathologist(SLP) is responsible for developing and implementing a Plan of Care for Home Health patients with communication and/or swallowing difficulties.
Professional Duties
- Evaluates home health patients and develops a plan of care based on home environment, patient identified needs, medical needs, and equipment needs.
- Performs initial and ongoing assessments including the OASIS when applicable.
- Identifies functional speech and communication defects and works with the patient/family to develop a plan of care that meets both the patient/family and agency goals.
- Recommends communication devices/aids as indicated.
- Participates in multi-disciplinary case conferences and QAPI when applicable
- Performs required direct/indirect patient care supervisory visits for Speech Pathologist Assistant per regulations.
- Provides health teaching to patient and/or caregiver regarding treatment and plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and/or caregiver
- Reports any change in the patient's condition to the Physician and the Therapy Manager.
- Evaluates the effectiveness of the plan of care and updates as needed.

Qualifications
- Master's degree in Speech Language Pathology. Doctorate degree in Speech Language Pathology, preferred. Current California Speech Language Pathologist License.
- Valid California Drivers License and proof of auto insurance.
- Experience working with geriatric patients, preferred.
- Ability to multi task and prioritize to meet deadlines.
- Good communication skills, detail oriented, critical thinking and problem solving skills.
- Excellent customer service and telephone etiquette skills
- BLS required within 60 days of hire
