Role Overview: The Speech-Language Pathologist evaluates, develops, and implements individualized treatment plans for patients with speech, language, cognition, voice, and dysphagia deficits in a home setting. They provide direct therapy, select and interpret diagnostic assessments, and modify intervention strategies based on ongoing evaluations.

Key Responsibilities

  • Assess patients' functional levels through diagnostic tests.
  • Develop and adjust personalized treatment programs aligned with patient needs and goals.
  • Deliver therapy services across various functional settings.
  • Coordinate care with interdisciplinary teams, including families and physicians.
  • Document evaluation results, treatment plans, and progress notes accurately.
  • Participate in program development and supervise clinical interns.

Qualifications & Skills

  • CCC-SLP certification from ASHA or active clinical fellowship with certification within one year.
  • State licensure as a Speech-Language Pathologist.
  • Preferred 1-3 years of experience in clinical settings such as hospitals, SNFs, outpatient, or home health.
  • Effective communication, visual acuity, and ability to work collaboratively.
