Brief Overview

Evaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting.

What you will do

  • Evaluates assigned patients through selection, administration, and interpretation of formal and informal diagnostic assessments to determine levels of functioning and intervention needs.
  • Demonstrates knowledge of age-specific factors and performs satisfactorily related to patient’s age from infant through geriatrics as noted on unit specific guidelines and competencies.
  • Develops and implements individualized treatment programs for patients needing services.
  • Establishes and modifies intervention goals and strategies based on ongoing evaluations.
  • Provides direct therapy services to patients using various functional settings and strategies.
  • Plans therapy within the patient’s individual physical, emotional, and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans.
  • Prepares written documentation such as evaluation results, individualized treatment plans, progress notes, and other reports.
  • Communicates effectively with patient, family, physician, and other disciplinary team members regarding status, progress, and needs.
  • Establishes rapport and works cooperatively with interdisciplinary team members and family/physician throughout the patient’s intervention program.
  • Participates in program development of the department and evaluates interns with students affiliating in various universities/colleges.

Essential Qualifications

The individual must be able to work the hours specified and perform each essential job function satisfactorily, including having adequate visual acuity and the ability to communicate effectively, hear, understand, and distinguish speech and other sounds.

Required Education, License/Certification, or Work Experience

  • CCC-SLP – American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year with certification received within one year of employment upon hire.
  • Speech-Language Pathologist – active license in the state of practice upon hire.

Preferred Education, License/Certification, or Work Experience

1‑3 years as a Speech Language Pathologist in a hospital, SNF, outpatient, and/or home health setting.
