Role Overview
Provides speech therapy services in a home setting, evaluating patients, developing individualized treatment plans, and delivering direct therapy to address speech, language, cognition, voice, and dysphagia deficits.
Key Responsibilities
- Assess patients through diagnostic assessments and interpret results to determine intervention needs.
- Develop, implement, and modify personalized treatment programs based on ongoing evaluations.
- Deliver therapy tailored to patients' physical, emotional, and cognitive capabilities, coordinating with other healthcare providers.
- Document evaluations, treatment plans, progress notes, and communicate effectively with patients, families, and team members.
- Participate in program development and supervise clinical internships with students.
Qualifications & Skills
- CCC-SLP from ASHA or current supervised clinical fellowship with certification within one year, or active state licensure.
- Ability to work flexible hours, communicate effectively, and maintain confidentiality.
- Preferred 1-3 years of experience in various healthcare settings such as hospitals, SNFs, outpatient, or home health.
