We are seeking a compassionate and clinically driven Registered Speech Language Pathologist (SLP) to join our rehabilitation team and provide individualized therapy services to residents with communication, cognitive, swallowing, and functional challenges.

The Right Candidate Will Bring:

  Master’s degree in Speech Language Pathology from an accredited program

  n

• n

  Current Speech Language Pathology license in the applicable state

  n

• n

  Certificate of Clinical Competence (CCC) or Clinical Fellowship Year (CFY) eligibility

  Long-term care experience preferred

Responsibilities

Speech Language Pathologists at Bear Mountain help residents improve communication, cognitive function, swallowing safety, and overall quality of life through individualized, evidence-based therapy.

As a Registered Speech Language Pathologist, you will:

Resident-Centered Therapy & Clinical Care

  Evaluate residents and develop individualized treatment plans based on their communication, cognitive, swallowing, and functional needs

  n

• n

  Provide skilled speech therapy services in accordance with physician orders, professional standards, and regulatory requirements

  n
n
• n

  Implement therapeutic interventions designed to improve communication, cognition, swallowing function, and independence

  n
n
• n

  Complete timely assessments, screenings, progress notes, and clinical documentation

  n
n
• n

  Monitor resident progress and adjust treatment approaches to support meaningful outcomes

  n
n
• n

  Educate residents, families, and caregivers on strategies to support continued progress and success

  n
n

Collaboration & Quality Outcomes

• n

  Partner with nursing, rehabilitation professionals, physicians, and interdisciplinary teams to support resident-centered care

  n

• n

  Participate in care planning and discharge planning activities

  n

• n

  Communicate effectively regarding resident progress, treatment goals, and recommendations

  Maintain a safe, organized, and supportive therapy environment

Qualifications

SLP: Speech-Language Pathologist:

n
• n

  M.A. or M.S. Degree in Speech-Language Pathology from an approved school required.

  n
n
• n

  State license to practice as a Speech-Language Pathologist required.

  n
n
• n

  Clinical Competence Certification from the American Speech and Hearing Association preferred.

  n
n
• n

  Ability to communicate well verbally and in writing required.

  n
n
• n

  Current, valid CPR/BLS credential.

  n
n
• n

  Qualities of maturity, diplomacy, and ability to work well with others required.

  n
n
• n

  Verification of qualifications by the State Department of Health

  n
n
• n

  Rehabilitation Therapies (license number/certification number) required.

  n
n
• n

  Ability to perform tasks to established standards of excellence required.

  n
n
• n

  This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
