Job Summary: Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our pediatric home health team in New York . In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.
Requirements
- Active NY State Speech-Language Pathologist License — required
- Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
- Basic computer literacy for documentation and communication
- Strong clinical and interpersonal communication skills
- Ability to work independently in a home health environment
- Reliable transportation for home visits preferred
Job Responsibilities
- Evaluate patients and develop individualized treatment plans based on patients’ needs and goals
- Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders
- Maintain accurate and timely documentation of patient care
- Collaborate with the clinical team to ensure coordinated and effective treatment
- Communicate effectively with office staff, patients, and caregivers
- Collaborate with healthcare professionals, including speech therapy assistants, nurses, and physicians, to ensure comprehensive care
