Role Overview:

The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation to patients in a skilled nursing facility setting.

Key Responsibilities:

  • Assess, establish, and modify functional treatment goals.
  • Implement and follow personalized therapy plans under supervision if licensed accordingly.
  • Chart patient progress accurately and timely, utilizing therapy software.
  • Collaborate with interdisciplinary teams to support patient care.
  • Participate in community health initiatives and maintain professional relationships.
  • Ensure adherence to safety, confidentiality, and legal standards.
  • Supervise and evaluate speech therapy assistants or CFYs if applicable.

Qualifications & Skills:

  • Graduate of an accredited speech therapy program (MS or MA).
  • ASHA certification (CCC) or eligibility for Clinical Fellowship Year (CFY).
  • Current state licensure and CPR certification.
  • Preferred experience in post-acute care; proficiency in Microsoft Office.
  • Strong decision-making, communication, and teamwork abilities.
  • Ability to perform physical tasks such as lifting and moving patients.
