Overview
Kingston Care Center of Fort Wayne, IN is seeking a dedicated Speech Language Pathologist (SLP) to join our skilled nursing and rehabilitation team. In this role, you’ll help residents strengthen communication, improve swallowing, and enhance cognitive abilities while working in a resident-centered environment in Fort Wayne, Indiana. Full-Time or Part-Time | On-Site | Indiana License Required
Responsibilities
- Evaluate, diagnose, and treat speech, language, voice, swallowing, and cognitive-communication disorders
- Develop and implement personalized treatment plans tailored to resident needs
- Provide one-on-one and group therapy sessions to support functional communication and safe swallowing
- Document assessments, daily notes, and progress accurately in the EHR system
- Collaborate with physicians, nurses, occupational therapists, physical therapists, and families to ensure coordinated care
- Educate residents, caregivers, and staff on communication strategies and swallowing techniques
- Ensure therapy practices comply with Indiana regulations and Kingston’s standards of care
Qualifications
- Active Indiana SLP license required
- Master’s degree in Speech-Language Pathology from an accredited program
- CCC-SLP preferred; CFY candidates considered
- Experience in skilled nursing, rehab, or long-term care preferred (new grads welcome)
- Strong clinical, documentation, and communication skills
