PRN Speech-Language Pathologist (SLP)

We are seeking a compassionate and dependable PRN Speech-Language Pathologist (SLP) to evaluate and treat patients with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. The SLP will develop individualized treatment plans, educate patients and caregivers, document progress, and collaborate with the interdisciplinary care team.

Responsibilities

  • Evaluate patients for speech, language, cognitive-communication, voice, fluency, and swallowing disorders.
  • Review medical histories, diagnostic results, modified barium swallow study findings, and other relevant information.
  • Develop and implement individualized plans of care based on evaluation findings and provider recommendations.
  • Treat conditions such as dysphagia, cognitive impairment, aphasia, dysarthria, apraxia, delayed language, stuttering, and voice disorders.
  • Recommend appropriate diet textures, liquid consistencies, swallowing precautions, and compensatory strategies within the SLP scope of practice.
  • Select alternative or augmentative communication systems and teach patients and caregivers how to use them.
  • Instruct patients in exercises and techniques to improve communication, swallowing, oral‑motor function, and respiratory support.
  • Educate patients and family members on treatment goals, communication strategies, swallowing safety, and carryover techniques.
  • Complete evaluations, treatment notes, progress reports, and other required documentation accurately and promptly.
  • Participate in care conferences, team meetings, and patient progress reviews as needed.
  • Communicate changes in patient condition and treatment recommendations to physicians and other members of the care team.
  • Collaborate with and provide guidance to therapy staff and other healthcare professionals.
  • Follow all applicable clinical, safety, regulatory, and organizational standards.

Qualifications

  • Current, valid Speech‑Language Pathologist license in the state of practice.
  • Graduate degree in Speech‑Language Pathology from an accredited program.
  • Current CPR certification.
  • Valid driver’s license and automobile insurance.
  • At least one year of rehabilitation experience preferred.
  • Long‑term care and/or home health experience preferred.
  • Homecare Homebase experience preferred.
  • Strong clinical judgment, communication, documentation, and time‑management skills.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.

Schedule

  • PRN/as‑needed
  • Schedule may vary based on patient needs and census.
  • Local travel may be required.

Physical Requirements

  • Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
  • Ability to assist with patient mobility and positioning.
  • Ability to lift, carry, push, or pull up to 100 pounds, with assistance or appropriate equipment when required.
