Job Overview
What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):
In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate, and a member of the interdisciplinary care team.
Please note: All opportunities at 21HHS require being in the field visiting patients in their homes.

Qualifications
- Current unrestricted CA Speech Therapist License
- Current BLS/CPR Certification
- Valid CA Driver's License
- Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury, and of the influence of culture on health care
- Knowledge of state and federal homecare regulatory guidelines preferred
- Experience in home health care highly preferred
- Experience with high acuity patients a plus
- Experience working with an interdisciplinary team highly desired
- Case Management fundamentals highly desired
- Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized
