Speech Language Pathologist (SLP)

We are seeking a Home Health Speech Language Pathologist (SLP) to provide personalized speech therapy services in patients’ homes, focusing on communication, swallowing, and cognitive impairments resulting from illness or injury.

Key responsibilities include:

• Conducting assessments, documenting progress, and ensuring patient goals are met
• Implementing therapy exercises for speech, swallowing, and cognition
• Educating patients and families on care plans, safety, and self-care practices
• Collaborating with a multidisciplinary home health team to deliver comprehensive care

Qualifications and skills required:

• Master’s or Doctoral degree in Speech Language Pathology and active license in Colorado
• At least one year of experience in speech therapy, preferably in home healthcare
• CPR certification and ability to pass background and drug tests
• Knowledge of home health regulations, strong clinical judgment, empathy, and excellent communication skills
