Countryside Care Center is looking for a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team. You'll play a key role in helping residents regain communication, cognitive, and swallowing abilities while working in a collaborative skilled nursing and post-acute care environment.
What You'll Do:
- Perform comprehensive speech, language, cognitive, and swallowing evaluations
- Design individualized treatment plans that promote functional independence
- Deliver evidence-based therapy for speech, communication, cognition, and dysphagia
- Monitor resident progress and update treatment plans as needed
- Collaborate with physicians, nursing staff, PTs, OTs, and families to provide coordinated care
- Maintain timely, accurate documentation in compliance with regulatory requirements

Qualifications:
- Active Illinois Speech-Language Pathologist license required
- Excellent communication and clinical decision-making skills
- Experience in skilled nursing or rehabilitation is preferred, but not required
- New graduates are welcome to apply
