Speech Language Pathologist - Long Term Care

Job Description

The Speech Language Pathologist (SLP) will provide comprehensive evaluation and therapeutic services for patients in a long-term care setting. The SLP will work closely with patients, families, and multidisciplinary team members to support communication and swallowing needs.

Responsibilities

• Conduct evaluations and assessments of patients' speech and language abilities.
• Develop and implement individualized treatment plans to improve patients' communication and swallowing capabilities.
• Collaborate with healthcare staff to ensure effective care coordination.
• Document patient progress and adjust treatment plans as necessary.
• Provide guidance and support to patients and their families regarding communication strategies.

Required Experience / Certifications / Licensure

• Graduate of an accredited college/university program in Speech Language Pathology.
• Current New York State license as a Speech-Language Pathologist.
• Eligibility for or possession of the Certificate of Clinical Competence in Speech Language Pathology from the American Speech-Language-Hearing Association (ASHA).
• Experience in long‑term care settings is mandatory.
• Proficient with MS Office and Epic/EMR software.
