South Pacific Rehab Services, a Hawaii-based provider of post-acute and long-term care rehabilitation, is seeking a Speech Language Pathologist to join our collaborative, patient-centered team. In this on-site role within skilled nursing and post-acute facilities, you will evaluate and treat adults with speech, language, cognitive-communication, and swallowing disorders.
Responsibilities include completing assessments, developing and implementing individualized treatment plans, providing evidence-based therapy, and documenting progress in compliance with regulatory standards. You will collaborate closely with PT, OT, nursing, and physicians to support functional independence and improve quality of life.
