The SLP will assess communication, cognition, and swallowing; develop individualized treatment plans; and deliver evidence-based therapy for post-acute and long-term care residents. You will collaborate closely with an interdisciplinary team, document in EMR, educate families and staff, and support quality, resident-centered outcomes in a fast-paced, compassionate environment.
