In this role, you will manage a diverse caseload across acute and/or outpatient settings, develop evidence-based care plans, and deliver individualized, patient-centered therapy. You will collaborate closely with physicians, nurses, rehab specialists, and families to optimize functional outcomes, ensure accurate EMR documentation, and support quality, safety, and performance improvement initiatives within our mission-driven, academic health system.
