Job Description Summary

Great opportunity for a Nurse Practitioner or Physician Assistant to join our Interventional Radiology Team at Decatur Memorial Hospital! This is a Specialty Care Advanced Practice Provider position. This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role. Under the direction physician or designee, provides coordination of their respective department or work area. Provides comprehensive health care to patients in collaboration with physicians and other medical staff including medical evaluation, counseling and referrals.

Licensure / Certification / Registry

• One of the following is required:
• Must possess and maintain licensing as an APRN as required by Illinois State Law;
• Must possess and maintain licensing as a PA as required by Illinois State law
• Appropriate certification as a APRN or PA, as determined by department.
• Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire).
• Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required.
• Valid CPR certification required.
• ACLS where required.

Experience

• Minimum of 1-year post-graduate as a mid-level provider preferred

• Additional compensation in the form(s) of participation in our annual incentive program are dependent on role. Pay is based on factors such as work experience, education, certification(s), training, skills, and competencies related to the role.
