To support a growing population health team, the part-time South Carolina Licensed Nurse Practitioner will provide telephonic and video-based follow-up care to patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination. Key responsibilities Manage telephonic outreach and scheduled visits for patients recently discharged from the hospital Coordinate care across various settings, including hospitals, pharmacies, and community providers Collaborate with nursing staff to achieve individual performance targets and contribute to population health goals Required qualifications Active South Carolina Nurse Practitioner license 5+ years of post-graduate clinical experience in Primary Care or Geriatrics Experience engaging Medicare and Medicaid populations with complex conditions Motivational interviewing skills for patient engagement Comfort with virtual care delivery and digital tools
