To support a pioneering healthcare team, a part-time, remote South Carolina Licensed Nurse Practitioner will conduct virtual wellness visits, health assessments, and collaborate with in-home clinical teams to deliver comprehensive patient care. Key responsibilities Conduct virtual adult and pediatric wellness visits and health assessments Collaborate with in-home team members to evaluate patient needs and coordinate care Provide comprehensive care management, including preventive care interventions and medication reviews Required qualifications Board Certified as a Family Nurse Practitioner Active Family Nurse Practitioner license in South Carolina Experience performing wellness visits Technologically savvy with proficiency in using electronic health tools Ability to work autonomously and exhibit professionalism and compassion in patient care
