Setting
Outpatient Clinic working with the geriatric population.
Responsibilities
- Conduct social work assessments to identify the psychosocial needs, preferences, and goals of patients, contributing to the development of personalized care plans.
- Actively participate in team meetings to collaborate on care plan creation and patient management.
- Provide social work interventions as outlined in patient care plans, including resource arrangement, care transitions, individual and group counseling, and case management.
- Partner with primary care physicians and other team members to ensure smooth transitions of care between settings, such as hospitals, skilled nursing facilities, and home care.
- Coordinate, facilitate, and lead care conference meetings to ensure effective communication and care coordination with patients, families, caregivers, and team members.
- Offer discharge planning and support when patients disenroll from the program.
- Other duties as assigned.
Qualifications
- Master’s Degree in Social Work (MSW).
- A minimum of one (1) year of experience working with a frail or elderly population.
- Experience coordinating and facilitating care conference meetings.
- Preferred experience working with individuals who have behavioral health and substance abuse issues.
