To support the Care Coordination program, the full-time temporary California Licensed Care Coordination Social Worker will develop individualized intervention plans, collaborate with an interdisciplinary team to ensure quality outcomes, and participate in quality improvement efforts while working remotely in California. Key responsibilities Develop and manage individualized intervention plans for members referred to the Care Coordination program Collaborate with the Care Coordination team to educate members and facilitate navigation of the managed care system Engage in Quality Improvement efforts to evaluate the effectiveness of the Care Coordination Program Required qualifications Master's Degree in Social Work or a related field Two years of social service experience in an acute care setting, community agency, or managed care environment Knowledge of case management principles, community resources, and Medi-Cal policies Ability to create and implement intervention plans and advocate for quality care Strong critical thinking and problem-solving skills
