The incumbent will work to synchronized service, including care and case management along the health care continuum. This incumbent is a champion and leader in the Tribal HUD VASH program.
The incumbent will provide program coordination at least 25% of the time to implement and maintain this new and growing program. This is accomplished in collaboration with all of the ICC's as well as care and case managers across the healthcare system. As a social worker, the incumbent demonstrates innovation, creativity, expertise and skill in an expeditious and efficient manner. The incumbent is accountable to the Homeless Program Manager in carrying out operational functions described above. The incumbent has clinical involvement in patient care by providing clinical case management along with the program management. Clinical social work services may be delivered in person or by electronic media, such as phone, VTel or home tele-health. Travel is required up to 75% and will include transporting patients in a non-commercial vehicle.
A valid driver's license is required. The position will be responsible for coordinating and implementing the Turtle Mountain Tribal HUD VASH Program. The incumbent will be attending National, VISN calls and training on Tribal HUD VASH programming. In addition, the position will be in a program development/implementation role. The incumbent will identify outcomes and measurements to highlight the success of the Tribal HUD VASH Program. They will monitor process outcomes for education, accountability opportunities and/or continuous process improvement.
