Social Worker (Program Coordinator)
The VA Long Beach Health Care System is recruiting for a Social Worker (Program Coordinator) to coordinate the Medical Foster Home (MFH) and the Contracted Adult Day Health Care (CADHC) Programs. The SWPC is responsible for maximizing psychosocial quality of life issues to assist Veterans to remain in their home or in a less restrictive level of care other than a nursing home and avoid hospitalization. Works closely with Home-Based Primary Care (HBPC) and Spinal Cord Injury (SCI) Home Care.
