Transitions Care is seeking a compassionate Social Worker/LSW to support hospice and palliative patients and their families. In this role, you will complete psychosocial assessments, provide counseling and crisis intervention, coordinate community resources, and guide families through goals-of-care and end-of-life decisions. You’ll collaborate closely with an interdisciplinary team, contribute to care planning, and document in the EMR.
