Qualifications and Description for a Social Worker LSW / CSW:
- PA Social Worker CSW / LSW License
- Conduct Social Service Assessments and Documents According to State and Federal Regulations
- Provide Psychosocial Assessments and Relevant Parts of Minimum Data Set (MDS)
- Counseling Residents
- Gerontology / Nursing Home Experience Preferred
