Position Highlights
- Full-Time
- Salary: $43.90 - $58.55 per hour (based on experience)
- Excellent PTO and 1199 Union Benefits
- Rotating weekends and holidays required
- Conduct biopsychosocial assessments and discharge planning.
- Provide counseling and crisis intervention for patients and families.
- Connect patients with community, financial, and social service resources.
- Collaborate with interdisciplinary healthcare teams.
- Advocate for patients' psychosocial and emotional needs.
- Facilitate referrals and support services as needed.
- Investigate and address cases involving suspected abuse or neglect.
Requirements
- Master's Degree in Social Work (MSW).
- NYS LMSW or LCSW required.
- Hospital experience preferred; new graduates welcome.
- Strong communication, counseling, and organizational skills.
- Knowledge of community resources and discharge planning.
