Assignment: 3 Month Contract

Location: Flint, MI 48532

Shift: 8 Hrs. Days 7:30 AM - 4:00 PM

Job description:

Generates and accepts Social Work referrals, applies casework process, performs department duties, documents activities and performs social work functions of counseling, referral/resource acquisition/and case consultation and coordination.

Responsibilities:

  • Accepts, screens and generated referrals.
  • Utilizes casework process to provide high quality and sound services to clients.
  • Subjects case activity to scrutiny of supervision and professional peers as a measure of quality assurance.
  • Utilizes the social work problem list as the reference for case activity.
  • Participates in coordinated and cooperative manner with allied health team and physician to maximize patient functioning and enhance departmental performance.
  • Participates appropriately in team conferences, patient meetings, interviews, and group sessions required.
  • Assesses need for and processes requests for charity care for special funds.
  • Completes statistical and patient care forms and documentation as appropriately requested, required.
  • Assumes responsibility for assigned caseload/units/programs and accompanying service delivery.
  • Expands and limits participation in case activity, team activity, and service delivery as appropriate.
  • Utilizes family and community agencies to problem solve and provide direct services as appropriate. Processes, assists with, and facilitates appropriate referral paper work and documentation.
  • Assists in orientation of new social work employees as requested or directed.
  • Provides peer consultation and review as requested or directed.
  • Produces work activity outputs, inputs, and outcomes; and documents as appropriate at expected levels.
  • Participates in Medical Center and/or departmental quality assessments and continuous quality improvement opportunities, goal setting, collection and evaluation of data, implementation of action plans, or related applications.
  • Performs other duties as required or requested.

Requirements:

  • Possess a Master’s degree in Social Work from accredited higher educational institution - Required
  • Michigan Licensure as Masters Level Social Worker (L.M.S.W.)
