Job Title: Renal Social Worker

Employment Type: Full-time

Shift : Mon-Fri, day shift

Location : Holy Cross Silve Spring

Position Purpose

Responsible for assessing, counseling, referral and supportive services to patients and their families in a manner consistent with professional standards and institutional mission and policies. Collaborates with physicians, medical professionals and other staff to ensure the quality of care given. Provides care for the adult or geriatric patient (as defined in the unit scope of care). Utilizes knowledge of the principles of growth and development appropriate to the patient's age to ensure that psychosocial needs of the patient are met. Involves appropriate parties (i.e. patient's family or guardian, outside agencies) in the psychosocial care of the patient as needed.

As a Renal Social Worker, you will:

  • Demonstrate adequate communication with the patient, the patient's family (when appropriate) and other members of the health care team.

  • Assess patient transportation needs.

  • Complete application for the Kidney Disease Program on patients without secondary insurance.

  • Provide ongoing contact with patients and billing departments to ascertain current insurance information.

  • Document assessment of patient needs in medical records.

  • Plan and participate in multidisciplinary patient care conferences on a biannual basis, or more frequently if needed. Inform Director and/or Coordinator of patient care information issues.

Minimum Qualifications:

  • Master's degree in social work from an accredited university

  • Current LCSW Licensure by the State of Maryland, or eligibility for licensure

  • 1+ years of experience

Pay Range :

  • $34.07 - $52.84 per hour

  • Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
