Position Summary
Columbus Community Hospital is seeking a compassionate and dedicated Social Worker to provide educational, psychosocial, and supportive services to patients within the SBH Unit. This position plays a key role in assisting patients and families by coordinating support services, facilitating counseling resources, and promoting positive patient outcomes through effective care coordination and community engagement.
Essential Duties and Responsibilities
  • Provide educational, psychosocial, and emotional support to patients and their families.
  • Assess patient needs and develop appropriate plans for support and intervention.
  • Coordinate support services, including counseling referrals and community resources.
  • Collaborate with healthcare providers and interdisciplinary team members to ensure continuity of care.
  • Assist patients in navigating healthcare and community support systems.
  • Advocate for patients and families while promoting access to available resources.
  • Facilitate communication between patients, families, and healthcare professionals.
  • Maintain accurate and timely documentation in accordance with hospital and regulatory requirements.
Qualifications
Education
  • Master's Degree in Social Work required.
Experience
  • Minimum of five (5) years of healthcare social work experience preferred.
Licensure/Certification
  • Nebraska State Certification as a Certified Master Social Worker (CMSW) required.
  • Licensed Master Social Worker (MSW ) and/or Nebraska State Certification as a Licensed Mental Health Practitioner (LIMHP) or DSW as applicable and required by state regulations.
Knowledge, Skills, and Abilities
  • Strong counseling and interpersonal communication skills.
  • Ability to coordinate services across multiple disciplines and community organizations.
  • Knowledge of healthcare systems, community resources, and patient advocacy principles.
  • Ability to maintain confidentiality and demonstrate professionalism in all interactions.
  • Excellent organizational and problem-solving skills.
