The Role
We are seeking an empathetic, resourceful, and dedicated Social Worker to join our interdisciplinary care team.
In this role, you will be the ultimate advocate for our residents and a vital lifeline for their families. You will manage the psychosocial well-being of our residents from admission through discharge, helping them navigate transitions, addressing emotional and mental health needs, and ensuring they have the resources required to thrive-whether they are here for short-term rehab or long-term care.
Key Responsibilities
  • Resident Advocacy & Psychosocial Support: Conduct initial and ongoing psychosocial assessments to identify residents' emotional, mental, and social needs. Develop individualized care plans to address those needs.
  • Admission & Transition Coordination: Help new residents and their families adjust to the community, guiding them through the emotional and logistical transitions of moving into a senior living environment.
  • Discharge Planning: Lead the discharge planning process for short-term rehabilitation residents, coordinating home health services, medical equipment, and community resources to ensure a safe transition home.
  • Crisis Intervention & Counseling: Provide supportive counseling and crisis intervention to residents and families facing difficult diagnoses, cognitive decline, or end-of-life adjustments.
  • Interdisciplinary Collaboration: Actively participate in care plan meetings, partnering closely with nursing, therapy, activities, and dietary teams to ensure holistic care.
  • Documentation: Maintain timely, accurate, and compliant progress notes and social service documentation in accordance with state and federal long-term care regulations.
Qualifications
  • Education & Credentials: Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution. An active Illinois LSW or LCSW license is highly preferred.
  • Experience: Minimum of 1-2 years of social work experience in a skilled nursing, long-term care, assisted living, or hospital setting. Strong knowledge of Medicare, Medicaid, and discharge planning resources is a huge plus.
  • The Heart: Deep empathy, exceptional listening skills, and a genuine passion for working with the geriatric population and their families.
  • Skills: Strong communication, excellent conflict-resolution abilities, and comfort working in a fast-paced healthcare environment.
