VitalCaring seeks a compassionate Social Worker to support home health and hospice patients and families. In this role, you’ll complete psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect patients with community resources. You’ll collaborate closely with an interdisciplinary clinical team to support safe transitions, chronic disease management, and end-of-life care.
