Position Description:

Under supervision of a master prepared social worker, provides counseling to families, individuals and groups. Works in collaboration with multidisciplinary teams, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff.

Additionally, this position is not eligible for remote or hybrid opportunities. All work is required to be performed onsite at TTEP.

Major/Essential Functions:

  • Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.

  • Interview patients, assess priorities and document case activity.

  • Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.

  • Assist clients/families with obtaining community assistance by referral to proper resources.

  • Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.

  • Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.

  • Remain current with all licensure, certifications and mandatory compliances and trainings required of this position.

  • Adhere to all policies, procedures and practices (Regents Rules, TTUS, HSECEP OPs, etc.).

  • Personally, and consistently, demonstrate, display and act in accordance with Texas Tech Health El Paso’s Values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork). Serve as a Value's leader while actively promoting and encouraging staff across the institution.

Preferred Qualifications:

  • Ability to provide counseling to individuals, families, and groups; psychosocial evaluation; advocacy; and referral to community resources.

  • Effective human relation skills.

  • Ability to communicate effectively in both verbal and written.

  • Working knowledge of community agencies and services available for the special needs of assigned patient population.

  • Bilingual in Spanish and English.
