Social Worker - SNF's Located In Brooklyn, NY
Salary: $80K to $90K range
Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
Provide information to the resident, and/or family member, on residents' rights and responsibilities
Qualifications:
- Must have BSW or MSW
- Must have experience as a Social Worker in a skilled nursing facility
- Must have experience in long-term care
- Must have strong organizational and case management abilities
- Excellent communication and interpersonal skills
