Homeless Veteran Case Manager

The Homeless Veteran Case Manager is responsible for providing case management and outreach for the programs and services that fall under the purview of Healthcare for Homeless Veterans (HCHV). Major duties include, but are not limited to the following:

  • The HV Case Manager provides case management and support to Veterans, according to the Veteran's current acuity stage of case management. They seek regular training, consultation and clinical guidance from the HCHV Program Supervisor. The Case Manager may assist with the day-to-day operations of the program, which may include assisting in writing policies, procedures and/or practice guidelines; data collection as needed; preparing reports; and implementing program processes.
  • The HV Case Manager will develop a working relationship with the designated Contracted Emergency Residential Services (GERS) / Low Demand Safe Havens (LOSH) point of contact to coordinate emergency housing when needed; and monitor psychosocial and stability.
  • The HV Case Manager will maintain a caseload; provide outreach services; verify Veteran status/eligibility for VA health care; accept the referrals for screening/admission to all HCHV programs; complete assessments to ensure appropriateness of placement into such programs; conduct comprehensive biopsychosocial assessments; complete the Columbia Suicide Severity Rating Scale Screen (C-SSRS) at admission, reassessment, or when clinically indicated; complete standardized tools, screenings and reminders; provide crisis intervention/stabilization; participate with the HUD-VASH team in clinical decisions for admitting Veterans ensuring use of acuity matrix for identification of case management needs.
  • The HV Case Manager will provide education to Veterans; instill hope; utilize low-demand services that are non-intrusive and are designed to establish trust and engagement with Veterans.
  • The HV Case Manager will have a fundamental knowledge of social work practice; an ability to identify, build, and work collaboratively with landlords, utility companies, and community agencies; an ability to accept direction; use knowledge of existing resources and creativity in assessing resources necessary to the successful implementation and maintenance of the program; knowledge of functional assessments of Veterans' ability to live independently, activities of daily living and instrumental abilities/deficits; knowledge of and commitment to total quality management and continuous quality improvement theory and practice.
  • Other duties as assigned.

Work Schedule: 8:00 am - 4:30 pm, Monday - Friday

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior (work experience) or military service experience.

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)
