Dunbar Center seeks a Social Services Director to oversee psychosocial care for residents in our skilled nursing and rehabilitation facility. The director leads assessments, care planning, care transitions, and discharge planning, ensuring residents’ emotional and social needs are met.
Responsibilities include supervising social services staff, coordinating with nursing, therapy, and medical teams, facilitating family conferences, supporting advance care planning, managing community resources and referrals, maintaining regulatory compliance and documentation, and promoting a resident‑centered culture of dignity, respect, and quality of life for seniors.
